Box-checking, as Coffeen uses the term, refers to a mode of research practice in which completing a task is equated with understanding its purpose. A research team member trained to complete an informed consent form knows the fields that need to be filled. A research team member trained to understand informed consent knows why participant understanding matters, what to do when a participant asks an unexpected question, and how to recognize when a consent conversation has not produced genuine comprehension.

“The research industry has built a very efficient system for producing compliance. What it has not built is a system for producing comprehension. Those are not the same thing,” Coffeen says.

The difference becomes visible under pressure. Protocol deviations, unexpected participant events, and sponsor queries that do not fit neatly into a standard operating procedure all require a research team to reason from principle rather than retrieve a procedure. A box-checking team lacks the foundation for that reasoning.

A Comprehension Mindset in a Compliance-Focused Industry

Coffeen is not arguing against compliance. He is arguing that compliance, as currently practiced, is insufficient on its own. The ICH-GCP guidelines that govern clinical research exist because the research community learned, over time and at considerable cost, what goes wrong when participant safety and data integrity are not protected with care. Those guidelines encode the lessons of that experience. A team that understands the lessons is more resilient than a team that only knows the rules.

“Compliance without comprehension is just paperwork. You can pass an audit on paperwork for a while. You cannot produce reliable science on paperwork alone,” he says.

The distinction plays out in tangible ways. A site with genuine GCP comprehension handles an audit differently from a site that has rehearsed audit responses. A team that understands why adverse events must be reported within specified timeframes takes those timeframes seriously in a way that a team following a procedure cannot fully replicate.

Earning the Right to Make This Critique

Coffeen’s critique of box-checking compliance is grounded in a career path that gives him standing to make it. He served in the United States Air Force, where he learned that the difference between a team that executes procedures and a team that understands mission is the difference between adequate performance and reliable performance under novel conditions.

He went on to earn his Doctor of Chiropractic degree from Life Chiropractic College West and practiced as a chiropractic physician in Lakewood Ranch from 2013 to 2017. In clinical practice, the gap between procedural knowledge and practical comprehension is immediately visible. A clinician who knows the protocol for treating a condition and a clinician who understands why the treatment works respond differently when the patient’s presentation does not match the textbook case. In clinical research, Coffeen found the same gap.

He pursued GCP certification and built his current work as a researcher and educator based in Lakewood Ranch, Florida, focused specifically on closing the distance between what the field trains and what the field actually needs.

Why Comprehension Matters

The stakes of comprehension in clinical research extend beyond individual site performance. The data produced at a clinical trial site feeds into regulatory submissions that shape drug approvals. The quality of that data depends on the quality of the decisions made by research teams across hundreds of individual interactions: a source document entry, a protocol deviation assessment, a conversation with a participant about what they are agreeing to.

“A site that checks boxes has a procedure. A site that understands why those boxes exist has a culture. Culture is what survives a sponsor visit, an FDA inspection, or an unexpected protocol deviation,” Coffeen says.

Culture, in this sense, is not a motivational concept. It is a practical one. A research site with a genuine compliance culture does not need to be reminded to do the right thing when no one is watching. The team has internalized why the right thing matters.

The Role of Training and Accountability

Coffeen argues that the shift from compliance to comprehension requires a different approach to training design. Current training programs, he observes, tend to lead with procedure. Here is the form. What they tend not to include is a clear, accessible explanation of why the requirement exists in the first place and what the requirement is protecting.

“We train people to fill out forms. We do not train them to understand what those forms are protecting. That is the problem I am trying to solve,” he says.

The fix, he argues, is not more training hours. It is different sequencing. Begin with mission and purpose. Build the procedural layer on top of that foundation. The comprehension-first approach takes more care upfront, but it produces a more durable result.

A Call for Change in Clinical Research

Coffeen is direct about what he wants the field to do differently. Not discard compliance, but build something underneath it: the understanding that makes compliance meaningful rather than merely performed.

“GCP is not a set of rules someone invented to make research harder. It is the accumulated knowledge of what goes wrong when you do not protect participants and data with care,” he says.

That understanding, Coffeen believes, is what the clinical research community in Lakewood Ranch and across the broader sector needs to invest in. The infrastructure for compliance is already there. What is needed now is the depth of comprehension that turns a compliant site into a trustworthy one. Compliance without comprehension is just paperwork.
